NICS heart failure forum report

This report captures the presentations, discussions and outcomes from the NICS heart failure forum 2004: improving outcomes in chronic care, held in Canberra in June 2004. The forum, attended by over 170 clinicians from all health care sectors, professions and jurisdictions, was organised as part of the NICS heart failure program.
