HREC forms

HREC forms can be used to register HRECs; notify AHEC about changes to HRECs or to the organisations responsible for the HRECs; and to terminate the registration of an HREC.

Introduction

These forms are designed to be saved electronically and then completed and posted to AHEC.

HREC Registration Form

This form is used by organisations to register a Human Research Ethics Committee (HREC) with the NHMRC.

Change of HREC Details Form

This form is used by organisations to change any of the details relating to the HREC or the organisation(s) responsible for the HREC.

HREC Closure Form

This form is used by organisations to terminate the registration of an HREC with the NHMRC. Registration may be terminated because the HREC is ceasing to operate or because the organisation no longer wishes to have the HREC registered with the NHMRC. HREC Annual Report Form

Information regarding the HREC Annual Report, including copies of HREC Annual Form and Appendix 1 to download, are located on the HREC Annual Reporting to NHMRC page.
